How it works

Fluorouracil is an analogue of uracil, a component of ribonucleic acid. The drug is believed to function as an antimetabolite. After intracellular conversion to the active deoxynucleotide, it interferes with the synthesis of DNA by blocking the conversion of deoxyuridylic acid to thymidylic acid by the cellular enzyme thymidylate synthetase. Fluorouracil may also interfere with RNA synthesis.

Side Effects

Very common: may affect more than 1 in 10 people : Ischemic ECG abnormalities (an insufficient supply of blood to an organ, usually due to a blocked artery) , Neutropenia (an abnormally low level of neutrophils in the blood) , Leucopenia (an abnormally low number of white blood cells in the circulating blood) , Anemia (condition in which the circulating red cell mass is insufficient) , Pancytopenia (a disorder in which the bone marrow greatly decreases or stops production of blood cells) , Decrease in the production of blood cells , High fever and a sharp drop in circulating granular white blood cells , The inflammation of the lining of the mouth and digestive tract , Pharyngitis (inflammation of the mucous membranes lining the pharynx) , Inflammation of the rectum or ? Loss of appetite ? Watery diarrhoea . Common:may affect up to 1 in 10 people : Angina pectoris (Severe pain in the chest associated with an insufficient supply of blood to the heart) , low white blood cells accompanied by fever Uncommon:may affect up to 1 in 100 people : Abnormality in the heart's rhythm , Heart attack , Myocardial ischemia (a loss of oxygen to the heart muscle) , Myocarditis (inflammatory disease of the heart muscle)Heart insufficiency ? Dilative cardiomyopathy (a type of heart disease in which the heart muscle is abnormally enlarged, thickened and/or stiffened) , Cardiac shock , Low blood pressure , Sleepiness , Dehydration , Bacterial infection in the bloodstream or body tissues , Gastrointestinal ulceration and bleeding, casting off the skin , Rhythmic motions of the eyes , Headache.

Drug Interactions

Methotrexate (an anti-cancer medicine) , Metronidazole (an antibiotic) , Calcium leucovorin (also called calcium folinate - used to reduce the harmful effects of anti-cancer medicines) , Allopurinol (used to treat gout)Cimetidine (used to treat stomach ulcers) , Warfarin (used to treat blood clots) , Interferon alpha 2a; brivudin, sorivudin and analogues (an antiviral) , Cisplatin (an anticancer medicine) , Phenytoin (used to control epilepsy/fits and irregular heart rhythm) , Vaccines , Vinorelbine (an anti-cancer medicine) , Cyclophosphamide (an anti-cancer medicine) , Levamisol (medicine used to treat worm infection) , Tamoxifen (an anti-cancer medicine).

Indication

Fluorouracil is used to treat many common cancers, particularly cancers of the large bowel and breast. It may be used in combination with other anti-cancer medicines or radiotherapy.

When not to Use

Do not use Fluorouracil • If you are allergic to Fluorouracil or any of the ingredients in list. • If you are in a seriously weakened (including nutritional) state due to long illness • If your bone marrow has been damaged by other cancer treatments (including radiotherapy) • If you know that you do not have any activity of the enzyme dihydropyrimidine dehydrogenase (DPD) (complete DPD deficiency) • If you have a potentially serious infection • If your cancer is non-malignant • If you are pregnant or you are breast-feeding • If you are taking or have taken in the past 4 weeks brivudine, sorivudine and similar drugs (antivirals) .

Pharmauracil Iv Precautions

Precaution

It is recommended that fluorouracil should only be given by, or under the strict supervision of, a qualified physician who is conversant with the use of potent antimetabolites and has the facilities for regular monitoring of clinical, biochemical and haematological effects during and after administration.

Pharmauracil Iv Warnings

Warning 1

Fluorouracil should be used with caution in patients with reduced renal or liver function or jaundice. Isolated cases of angina, ECG abnormalities and rarely, myocardial infarction have been reported following administration of fluorouracil. Care should therefore be exercised in treating patients who experience chest pain during courses of treatment, or patients with a history of heart disease.

Warning 2

Cases of encephalopathies (including hyperammonaemic encephalopathy, leukoencephalopathy) associated with 5-fluorouracil treatment have been reported from post-marketing sources. Signs or symptoms of encephalopathy are altered mental status, confusion, disorientation, coma or ataxia. If a patient develops any of these symptoms withhold treatment and test serum ammonia levels immediately. In case of elevated serum ammonia levels initiate ammonia-lowering therapy.

Warning 3

All patients should be admitted to hospital for initial treatment.
