How it works
Gentamicin, an aminoglycoside antibiotic, is bactericidal. Gentamicin passes through the gram-negative membrane in an oxygen-dependent active transport. As oxygen is required, this is why aminoglycosides are not effective in anaerobic bacteria.

Side Effects
Vestibular and auditory damage , nephrotoxicity , hypomagnesaemia , blood dyscrasias , purpura , nausea , vomiting , stomatitis , signs of liver dysfunction , antibiotic associated colitis , atrophy at injection site , ototoxicity , neuromuscular blockade.

Do not use if you are allergic to any of the ingredients in the list.
Dosage
Adults : Upto 5mg/kg IM or IV daily in three divided doses for 7-10 days. Children : Upto 2 wk ,3mg/kg twelve hourly ; over 2 wk , 2mg/kg eight hourly. Both IM or IV for 7-10 days. Bone Infection : IM/IV : Adults : 3mg/kg/day given every 8 hr. Max : 5mg/kg/day in 3-4 doses . Reduce to 3mg/kg/day as soon as clinically indicated. Children : 2-2.5mg/kg every 8 hr. Infants/Neonates : 2.5mg/kg every 8 hr. 1 wk or less : 2.5mg/kg every 12 hr. Lower Respiratory Infection & Meningitis & Septicaemia : IV : Adults : 3mg/kg/day given every 8 hr. Max : 5mg/kg/day in 3-4 doses . Reduce to 3mg/kg/day as soon as clinically indicated . Children : 2-2.5mg/kg every 8 hr . Infants/Neonates : 2.5mg/kg every 8 hr. 1 wk or less : 2.5mg/kg every 12hr. OR As directed by your physician.

Precautions
To avoid adverse events, continuous monitoring (before, during and after) of renal function (serum creatinin, creatinin clearance), control of function of vestibule and cochlea as well as hepatic and laboratory parameters is recommended.
Warning 1
Ototoxicity has been recorded following the use of gentamicin. Groups at special risk include patients with impaired renal function, infants and possibly the elderly. Consequently, renal, auditory and vestibular functions should be monitored in these patients and serum levels determined so as to avoid peak concentrations above 10mg/l and troughs above 2mg/l when administering Gentamicin twice daily and 1mg/l for a once daily dose.
Warning 2
As there is some evidence that risk of both ototoxicity and nephrotoxicity is related to the level of total exposure, duration of therapy should be the shortest possible compatible with clinical recovery. In some patients with impaired renal function there has been a transient rise in blood-urea-nitrogen which has usually reverted to normal during or following cessation of therapy. It is important to adjust the frequency of dosage according to the degree of renal function.

Gentamicin should only be used in pregnancy if considered essential by the physician .
