How it works
The precise mechanism by which Lacosamide exerts its antiepileptic effects in humans remains to be fully elucidated. In vitro electrophysiological studies have shown that lacosamide selectively enhances slow inactivation of voltage-gated sodium channels, resulting in stabilization of hyperexcitable neuronal membranes and inhibition of repetitive neuronal firing. Lacosamide binds to collapsin response mediator protein-2 (CRMP-2), a phosphoprotein which is mainly expressed in the nervous system and is involved in neuronal differentiation and control of axonal doutgrowth. The role of CRMP-2 binding in seizure control is unknown.
USAGE AND SAFETY
The adverse events most commonly (>1% in the Lacosamide total group and greater than placebo) leading to discontinuation were dizziness, ataxia, vomiting, diplopia, nausea, vertigo, and vision blurred.
Medicines to treat heart problems , carbamazepine, lamotrigine or pregabalin , medicines used to treat certain types of irregular heart beat or heart failure , fluconazole, itraconazole or ketoconazole , ritonavir , clarithromycin or rifampicin , a herbal medicine used to treat mild anxiety and depression called St. John’s wort.
Lacosamide is indicated as a monotherapy in the treatment of partial-onset seizures in patients with epilepsy aged 17 years and older.

Antiepileptic drugs (AEDs), including Lacosamide, increase the risk of suicidal thoughts or behavior in patients taking these drugs for any indication. Patients treated with any AED for anyindication should be monitored for the emergence or worsening of depression, suicidal thoughts or behavior and/or any unusual changes in mood or behavior.
Treatment with lacosamide has been associated with dizziness which could increase the occurrence of accidental injury or falls.
Prolongations in PR interval with lacosamide have been observed in clinical studies. Lacosamide should be used with caution in patients with known conduction problems or severe cardiac disease such as a history of myocardial infarction or heart failure.
Lacosamide is not recommended for use in children & adolescents below the age of 16. The safety and effectiveness of Lacosamide in pediatric patients <17 years have not been established.
