How it works

Colistin is a cyclic polypeptide antibacterial agent belonging to the polymyxin group. Polymyxins work by damaging the cell membrane and the resulting physiological effects are lethal to the bacterium. Polymyxins are selective for aerobic Gram-negative bacteria that have a hydrophobic outer membrane.

Side Effects

Gastrointestinal: Gastrointestinal upset Nervous System: tingling of extremities and tongue, slurred speech, dizziness, vertigo, paresthesia, and seizures ; Integumentary: Generalized itching, urticaria and rash ; Body as a Whole: Fever and anaphylaxis ; Laboratory Deviations: Increased blood urea nitrogen (BUN), elevated creatinine and decreased creatinine clearance ; Respiratory System: respiratory distress and apnea ; Renal System: Nephrotoxicity and decreased urine output .

Drug Interactions

Kanamycin, streptomycin, dihydrostreptomycin, polymyxin, neomycin , curariform muscle relaxants (either tubocurarine, succinylcholine, gallamine, decamethonium and sodium citrate .

Indication

It is indicated for the treatment of acute or chronic infections due to sensitive strains of certain gram-negative bacilli. Particularly indicated when the infection is caused by sensitive strains of P. aeruginosa. This antibiotic is not indicated for infections due to proteus or neisseria. Sodium colistimethate has proven clinically effective in treatment of infections due to the following gram-negative organisms: A. aerogenes, E. coli, K. pneumoniae and P. aeruginosa.

When not to Use

It is contraindicated in patients with a history of sensitivity to the drug .

Kolisod 150Mg Precautions

Precaution

Colistimethate sodium should be used with extreme caution in patients with porphyria. Antibiotic-associated colitis and pseudomembranous colitis have been reported with nearly all antibacterial agents and may occur with colistimethate sodium. They may range from mild to lifethreatening in severity. It is important to consider this diagnosis in patients who develop diarrhoea during or after the use of colistimethate sodium. Discontinuation of therapy and the administration of specific treatment for Clostridium difficile should be considered. Medicinal products that inhibit peristalsis should not be given.

Kolisod 150Mg Warnings

Warning 1

Transient neurological disturbances may occur. These include circumoral paresthesias or numbness, tingling or formication of the extremities, generalized pruritus, vertigo, dizziness, and slurring of speech. For these reasons, patients should be warned not to drive vehicles or use hazardous machinery while on therapy.

Warning 2

Since sodium colistimethate is eliminated mainly by renal excretion, it should be used with caution when the possibility of impaired renal function exists. The decline in renal function with advanced age should be considered.

Warning 3

Caution is advised when administering colistimethate sodium to infants < 1 year of age as renal function is not fully mature in this age group. Further, the effect of immature renal and metabolic function on the conversion of colistimethate sodium to colistin is not known .
